work backwards... you want about 2.5 gallons of wort when all is said and done (so you can use that other .4ish gallons to contain the hop/break material), so you should have about 3.5 gallons at preboil. so, you'll need to calculate the grain absorption factor... it's easy... (lbs of grain in grain bill)X.2)... then add that number to the preboil volume of 3.5 and you'll see how much water you'll need to start with. confused? lol

on my system for my last recipe, I started with 4 gal of water and had 2.25 into the fermenter, using all the wort in the kettle. 6.6 # of grain on that recipe. Start with 4 gallons and go from there.

that sounds like a bit too much to me, just based on what i've used.
if you go with that amount, make sure you can fit that much grain and water. 4.25 gal of water and 6.6# of grain was all I could fit in a 5 gal pot. google "can I mash it" for a calculator

as far as I know, water levels won't affect mouthfeel. People use anywhere from 1qt/lb to 7 or 8 gallons for a full 5 gallon batch. Mash temps are key.

if you were asking me Banjo, I don't use a ratio. I did once or twice, but did not find the outcome any different than simply going up to 4.25 gal for most average brews. with a smaller or larger grain bill, I would scale up or down a bit (and always err on the high side...it is a lot easier to extend the boil IMO!) and like Jon suggested (he ain't BOTM for nuthin'!) just mash in the high range to get that fuller body. a lot of "real" breweries use this technique as opposed to adding carapils or other dextrins. I wanna say Red Hook and Lagunitas (from a highly respected souce: BN Sunday Session haha!), but I am not 100% certain if I am remembering that correctly. the technique yes, the brewery(ies) maybe
